Leave of Absence Policy:

• I understand that my leave of absence is subject to the PDC PEACE PLAN Leave of Absence Policy, which includes the following:
• All leaves of absence must be approved by the employee's supervisor and HR/Admin
• Employees must provide adequate notice for leaves of absence, whenever possible.
• Employees must ensure that their work is up to date and completed before taking a leave of absence.
• Employees must be available by email or phone during their leave, unless otherwise agreed upon by their supervisor and HR/Admin.
